1. FISSURE – IN – ANO

About the disease
A fissure contains a crack in the skin-lined part of the anal canal, which often shows a considerable reluctance to heal. It is mostly encountered in young and middle age, sometimes in early childhood or infancy. It is a common disease of the anus common in both sexes, which causes an amount of suffering out of proportion to the size of the lesion. Symptoms & Diagnosis

Excruciating Pain – in the anus during and after defecation described as a sharp cutting or tearing sensation during actual passing of motion and continuing as a less severe burning or discomfort for hours after defecation. 
Bleeding – invariable symptoms of fissure is very common; usually bleeding is quite slight and amounts to a little more than streaking of motion occasionally the loss is quite profuse and may cause anemia.
Swelling – some patients become aware of a large sentinel tag and complain of a painful external pile.
Discharge and Pruritus – much discharge leads to soiling of underclothes, and to increased moisture of the perianal skin, with resulting pruritus ani – Urinary symptom-disturbances of micturition, either dysuria and retention or increased frequency.

Investigation

2. FISTULA – IN – ANO

About the disease

Fistula indicates chronic process of Perianal abscess which is an acute Inflammatory event. It is defined as abnormal communication between any two Epithelium-lined surfaces. Fistula-In-Ano is an abnormal tract or cavity communicating with the rectum or anal canal by an identifiable internal opening. Symptoms & Diagnosis

Patients with a fistula -in-ano will recount a history of an abscess that has been drained surgically or drained spontaneously. In addition, patients will complain of drainage, bleeding, pain with defecation, or decrease in pain with drainage.

Cause

It is a chronic process of an abscess. Anal gland infections and infected fissure-in-ano, are the common causes. It may be secondary to other intestinal diseases.

Investigation

Investigations are done to find out the track and the internal opening of the fistula, ultrasound examination using rectal probe, fistulogram, and most reliable being probing with a metal probe.

Treatment

The principle behind the fistula surgery is to eliminate the fistula, prevent recurrence, and preserve sphincter function. The treatment being, Fistulotomy, an outpatient procedure done under local anesthesia where the fistula tract is laid open and the fistulous track is frozen to ensure complete destruction.

3. PERI – ANAL ABSCESS

About the disease
This results from obstruction of the anal glands and ducts which are located around the anal canal.

Symptoms & Diagnosis
Pain,swelling, and fever are hallmarks associated with an abscess. Tenderness and possible fluctuation may be seen.

Cause
Infection of anal glands, infected fissure-in-ano, and secondary to other intestinal diseases.

Treatment
Based on the size and severity of the abscess, under local anesthesia, drainage with primary fistulotomty and cryo-ablation of the cavity is done.
Patient feels relief almost immediately.

4. PILES/ HEMORRHOIDS

About the disease

Piles / Hemorrhoids are displaced anal cushions. The cushions are normal structures that have a rich arterial supply leading directly into distensible venous spaces. They help seal the upper anal canal and contribute to continence.

Symptoms & Diagnosis

Bleeding and prolapse, bright red blood dripping into and splashing around the pan after bowel action suggests hemorrhoids. Swelling of cushion in the anal canal which disrupts a smooth flow of the bowel action.

Bleeding is more of a problem in younger patients as the sphincter tone is higher, and it may occasionally cause anaemia.

About the disease
A pilonidal cyst is an abnormal pocket in the skin that usually contains hair and skin debris. A pilonidal sinus is almost always located near the tailbone at the top of the cleft of the buttocks.

Cause
Pilonidal sinus usually occurs when the hair punctures the skin and then becomes embedded. If a pilonidal sinus gets infected, the resulting abscess is often extremely painful. The sinus can be drained with a small incision or removed surgically.

Investigation

• MRI sinogram/fistulogram to rule out the connection to underlying bony structures.
• Surface Ultrasonography.
• Blood tests

Surgery

Under local anesthesia, the tract is identified and excision with cryo ablation is done.
Patient is kept for observation post surgery and can go home the very same day.
